The Brain Injury Rehabilitation Service (BIRS) at the Royal Rehabilitation Centre Sydney is developing a new Rehabilitation Case Management (RCM) model aimed at better meeting the needs of clients during the transitional phases of rehabilitation. In the previous model, the Rehabilitation Case Manager became involved approximately 2 weeks prior to inpatient discharge. Many clients involved in rehabilitation have identified that the transition points were the most stressful part of their journey and in some cases resulted in a decline in physical or psychological function. The BIRS RCM model aims to provide consistency throughout an individual's rehabilitation program and to build rapport and trust with both the client and their loved ones. In the new model, each client will be allocated a rehabilitation case manager on admission to the Brain Injury Unit, this rehabilitation case manager will work with the client and their family throughout their admission and will transition with the client into the community. Clients and families who are aware of the initiative have expressed how helpful they believe the new model would have been during their transition phases. The new model is due to be rolled out in April 2011.
